Background Investigation

Who:

All Partners and USDA Employees.

What:

Background Investigations are required for all persons that are using USDA equipment or connected to the USDA network.

When:

Partner’s Background Investigations must be submitted or completed by September 15, 2006.

How:

Step 1 -  Resume

We will need your current resume covering a five year period; using month/year format and complete address.

Step 2 - Download applicable fillable forms with instructions.

The following forms and brochures require Adobe Acrobat or Microsoft Word.

SF-85 (PDF; 335 KB) – Questionnaire for Non-Sensitive Positions

SF-86A   (PDF; 166 KB) - Continuation Sheet for Questionnaires

SF-85P   (PDF; 520 KB) - Question for Public Trust Positions - For Contract Employees only - in place of SF-85.

OF-306   (PDF; 92 KB) – Declaration for Federal Employment - an Office of Personnel Management requirement for all employees, contractors, and partners.

Fair Credit Release  (PDF; 10 KB) - Fair Credit Release

IRM-02  (PDF; 26 KB) – Computer User Security Agreement– to be signed by user and NRCS District Conservationist or Supervisor

IRM-03  (PDF; 384 KB) – Request for User Access to ITS Resources - to be completed by NRCS District Conservationist

Step 3 -  Finger Print Card

For NRCS Employees and Partners call your zone office to obtain the SF-87 – Fingerprint Card-RV February 2002 or April 2006.
Remember to complete all applicable boxes.

For Contract Employees contact Carla Fields to obtain the SD-258 - Fingerprint Card-RV May 1999.

Have your finger prints taken by your local  Police or Sheriff’s Office.  The cost to the employee for finger printing could vary from 0 to 20 dollars. 

Step 4 - Return all information to State Office for processing.

Print your completed fillable forms, sign them, and attach the finger print card and your current resume.

Step 5 - Read and comply with this important security information and complete security training listed below.

Complete the USDA Security Literacy and Basics FY 2006 and USDA Privacy Basics at AgLearn Web site within sixty days of Employment.
